Who is collecting and holding your information?

Medical Fitness NZ, operated in association with The Exercise Room, collects the information. It is held by Medical Fitness NZ / The Exercise Room and the contracted systems used to receive and manage enquiries.

What we collect

The Founding Access form asks for your name, email address, mobile number, services of interest, and an optional description of what you would like help achieving. Your answer may contain information about your health, fitness, recovery or physical capability.

Please only provide information relevant to your enquiry. Do not use the form for urgent medical information or emergencies.

Why we collect it

We use the information to:

• respond to your Founding Access enquiry;
• understand your interests and discuss whether our services may be suitable;
• arrange a Discovery Consultation or opening appointment if you choose;
• send relevant opening, programme, service and event information; and
• maintain an appropriate record of our communications with you.

We will not use health information supplied through this form to diagnose or treat a condition.

Providing information is voluntary

You do not have to submit the form. If you do not provide the required contact details, we will not be able to respond to or progress your Founding Access request. The goals field and interest selections are optional.

Who may receive it

Your information may be seen by authorised Medical Fitness NZ / Exercise Room team members who need it to respond to your enquiry. It may also be processed by contracted providers that support our website form, email, hosting or enquiry-management systems in order to provide those services.

We may also disclose information where you authorise us to, where disclosure is required or permitted by law, or where it is necessary to prevent or lessen a serious threat to health or safety.

We do not sell information submitted through this form.

Storage, security and retention

We use reasonable administrative and technical safeguards designed to protect enquiry information from loss, unauthorised access, misuse or disclosure. Information is retained only for as long as it is reasonably needed for the purposes above or to meet applicable legal and record-keeping requirements, then securely deleted or de-identified.

Access and correction

You may ask to access the personal or health information we hold about you and request correction if you believe it is inaccurate. You may also ask us to stop sending opening or service updates.
